Volunteer Role: Temporary, Health Equity Questionnaire Support Volunteer
Shift: 6-month commitment, 3-4 hours per week

Organization
Durham Community Health Centre (DCHC) is a registered, charitable organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.
DCHC also provides equity-based programs and services that focus on priority populations such as Indigenous, Black, the 2SLGBTQI Community, Newcomers to Canada, Seniors, and Unattached patients (i.e., those without a family doctor), to name a few. DCHC ensures Durham community members receive not only sick care, but preventive care as well.
At DCHC, we are a team-based interprofessional group of staff including physicians, nurse practitioners, nurses, counselors, dietitians, outreach workers, medical secretaries, and other administrative staff. We always place every client at the centre of our approach to care, based on their needs. We address these needs through integrated clinical and wellness care and health education.

Position Overview:
We are seeking enthusiastic and dependable volunteers to support our team in administering and completing our Health Equity Questionnaire with clients. This questionnaire helps us gather essential information to better understand and meet the needs of our clients and community. However, clients may find the form difficult to navigate, time-consuming, and feel uncertain about the purpose of specific questions. With volunteer support we hope to improve the client experience and reduce staff workload.
As a Health Equity Questionnaire Support Volunteer, you will work directly with clients to guide them through the form, clarify questions, and ensure responses are completed accurately and respectfully.
Responsibilities:
Welcome clients upon arrival and guide them to the appropriate person or location.
Assist clients in reading and completing the Health Equity Questionnaire.
Explain questions in a clear and respectful manner when needed.
Follow-up with clients to ensure completion of the Health Equity Questionnaire three (3) days prior to the clients initial appointment.
Assist clients in signing in and completing forms on Kiosks.
Ensure completion of the Health Equity Questionnaire when returned.
Create a welcoming and supportive environment for all clients.
Ensure Health Equity Questionnaire is periodically updated for clients.
Record and track form completion.
Report on any issues or client concerns to staff.
Maintain confidentiality and follow all privacy policies and guidelines.
Qualifications:
Strong verbal communication skills.
Comfortable working with individuals from diverse backgrounds.
Respect confidentiality and follow procedures.
Ability to travel to assigned site.
Preference will be given to candidates with medical administration training.
