Job Description
As an important member of the care team and under the supervision of the clinical team, the Health Care Aide will provide patient-inspired care and perform activities of daily living for individuals whose conditions/diagnoses are less complex and more predictable.
Accountabilities:
Perform direct patient services tasks including: assisting with patient activities of daily living, mobilization and transport
Collaborate with the patient and team to prevent decline in levels of function and ADL
Report changes in the patient's physical, cognitive and or emotional condition immediately to a nurse
Listen to and communicate patient concerns to appropriate team members
Participate in bedside report and team huddles to identify and prioritize patient care needs
Document care interventions on unit specific documentation forms and in the electronic health record
Perform patient rounds to assess the safety and comfort of patients
Daily restocking of supplies in patient care rooms and supply storage areas
Contribute to the maintenance of a safe work environment
Qualifications
Personal Support Worker certificate required from an accredited institution
Basic Knowledge of aseptic techniques
Current CPR
Recent related experience working with patients in a medicine and/or rehabilitation setting
Recent experience working in a highly technical patient care environment
Excellent organizational and prioritization skills
Excellent interpersonal, communication and comprehension skills
Flexibility to meet unit needs
Ability to work well with team members
Basic computer skills and knowledge of Meditech system
Excellent attendance, punctuality and work record
Ability to travel between and work at all hospitals as required
Able to handle physical requirements of the job (ability to lift at least 25-30 pounds)

Additional Information
Currently, days/evenings, 8 and 12 hours including weekends and statutory holidays (subject to change in accordance with operational requirements)
Salary:
Minimum: $30.76 per hour
Maximum: $31.38 per hour
