Position Summary:
Reporting to the Clinical Manager responsible for Flow, Mental Health Program, this role complements existing Admission Coordinator roles currently in place at CVH and MH sites and will be the key contact for bed flow and capacity within the Mental Health Program. The Mental Health Bed Flow Coordinator will serve as a clinical liaison within the program by providing cross-site operational leadership and support to proactively facilitate flow and patient transfers within Mental Health, with a particular focus on reducing ED length of stay (LOS) for admitted patients. Our shared goal is to safely facilitate patient flow organizationally and to mobilize teams to achieve throughput, create capacity and achieve success while providing an exceptional experience for patients, families and staff.

Together we will create high quality services that support people at each transition across the care continuum, and effectively links with community mental health services to create a more integrated system of mental health care for our community.
The ideal candidate is a reliable, collaborative team member who works closely with Physicians, Bed Allocation, Charge Nurses and unit-based teams to facilitate timely admissions, transfers and repatriations while maintaining a high-level perspective of site-specific operations and cross-site activity. Each understands the demand, capacity and practical challenges of a hospital system and works to ensure patients receive the right care, in the right place, at the right time.
Key Responsibilities:
In collaboration with the Clinical Manager, develop daily flow strategy that is responsive in real time to competing capacity demands and changes in unit/patient acuity
Act as cross-site resource for capacity and flow, to provide leadership and support to Charge Nurses/Staff, Bed Allocation, Physicians and Patients/Families
Provide co-ordination of patient admissions, transfers, and repatriations utilizing EPIC, standard operating procedures (SOP), protocols and situational awareness of each unit’s status within the Mental Health Program
Develop and maintain effective communication with the mental health physician leaders, inpatient clinical leaders and charge nurses in order to facilitate flow and capacity planning
Pre-assign patients to units with bed desk
Liaise with bed flow and bed desk regarding any patients that require transfer to medicine
Support CIT/EDMH nurses when on-site to receive consent from patients, and package out any packages to external hospitals
Collaborate with CL/CNs and IPAC to coordinate bed moves on the unit to help minimize blocked beds, optimize use of locked rooms and support with bed transfers to units
Lead repatriation requests to external hospitals from inpatient and ED.
Assess and make recommendations for improvement, when processes are stalled or less effective than expected in facilitating flow
Ensures follow-up and communication with inpatient unit managers on relevant issue(s), and escalations
Liaise with ECT/CL nurses to support any required patient transfers based on bed availability or placement decisions required
Work with the Clinical Leaders, Program Admin and Officer in Charge regarding Form 10's and follow-up for any that are outstanding
Support with the daily 8:40 mental health bed meeting and daily GTA mental health call (as needed)
Attend cross-site rounds in ED and on the inpatient units in order to inform daily bed flow planning, as well as to support in identifying and problem-solving discharge barriers for long stay patients
Qualifications:
Undergraduate degree in a health care discipline (nursing preferred)
Current member and in good standing with a professional college required
Minimum 3-5 years recent clinical experience in an acute care hospital and mental health setting
Proven leadership abilities
Strong facilitation and conflict resolution skills to help alleviate barriers
Ability to think critically to respond in real time to a rapidly changing environment
Ability to assess risk, and effectively utilize resources to support patient acuity while considering the global needs of the organization
Ability to be self-directed and thrive in a dynamic environment
Role models composure; Able to remain calm under pressure
Demonstrates positive, collaborative working relationships with interdisciplinary health care teams
Ability to establish rapport to achieve excellent customer service
Ability to actively listen, engage, collaborate, negotiate, facilitate and entertain creative solutions in order to enhance patient flow organizationally and mobilize teams to achieve throughput
Actively participates in and demonstrates support for continuous quality improvement initiatives
Excellent verbal and written communication skills
