Job Description: Reporting to the Manager of Stewardship and Clinical Appropriateness Dept, the Nurse Navigator is responsible for the assessment of clinical requests for patient needs for acute pain, Acute on Chronic Pain and exacerbation of chronic pain. Optimal pharmacological prescribed therapy, psycho- social aspects, barriers to discharge, and to determine appropriateness and alignment with the Opioid Stewardship criteria, monitoring and measuring outcomes for ongoing treatment. The Nurse Navigator will assist with the development and maintenance of a patient and prescriber needs, and will create and develop learning tools to educate, implement, evaluate, co-ordinate care and monitor outcomes of this program. The incumbent collaborates with multidisciplinary team members and liaises with health professionals and agencies within and external to the SHA to facilitate high quality care for Saskatchewan Stewardship patients.

Experience
Minimum of three (3) years administration experience.
One (1) year experience in adult education or a teaching role
Requires experience in program/resource development related to needs assessment, planning, program design, implementation, and evaluation
Education
Bachelor of Science in Nursing (BScN)
Licenses
Basic Life Support Certificate (BLS)
Valid driver's license
