Job Description: As a member of Mental Health and Addiction Services, the Community Mental Health Liaison Nurse is an integral member of a multidisciplinary team that will address the issues of complex clients who have a primary diagnosis of a major mental illness. This position will work closely with the psychiatry inpatient unit and the transition team to provide assistance with clients transitioning from hospital back into the SHA housing partnership community beds. Responsibilities include intensive case management and stabilization, medication management, crisis intervention, counseling, goal planning, establishment of community supports through consultation with community agencies, student mentoring, documentation, and client/family education.

Experience
Five years recent experience in community mental health or relevant setting required.
Education
Bachelor of Psychiatric Nursing (BPN)
Licenses
Basic Life Support Certificate (BLS)
Cardiopulmonary Resuscitation Certificate (CPR)
Other Education and Training
Nursing diploma is acceptable
Workplace Assessment & Violence Education (WAVE)
Psychiatric Nursing diploma is acceptable
Bachelor of Nursing is acceptable
