Summary:
Health Hubs are a key part of Trillium Health Partners’ strategy to expand access within a constellation of care built around people. Health Hubs expand access, quality, and sustainability by integrating services around patients’ needs through innovative partnerships and a population health approach. The Health Hubs team leads the advancement of opportunity assessment and planning for hubs of integrated care that will improve population health in our community including initiatives and projects that expand access, improve outcomes, advance integrated models of care and reduce capacity pressures across the system. The Health Hubs team is accountable for planning, developing and implementing strategic projects that advance a multi-year roadmap that includes Seniors Campus of Care, hospice developments, community-based ambulatory and surgical centres, and emerging strategic partnership opportunities.
Description:
We are recruiting for one Master’s Co-Op role within the Health Hubs team. The successful candidate may also have opportunities to work across teams, depending on interest.
Compensation Range: $24 – $26 per hour (commensurate with experience)
The Health Hubs Associate will:
Enable coordination, communication and controls in the delivery of projects of various sizes and scopes
Support planning and development related to key initiatives
Support the delivery of projects, processes and best practices designed to achieve the goals identified in the hospital’s strategic plan
Develop and deliver innovative presentations, reports, and proposals to all levels of the organization
It is anticipated that the Health Hubs Associate will:
Support a variety of internally- and externally-facing cross-functional projects simultaneously
Support strategic plan delivery at all levels of the organization
Engage and communicate with stakeholders at all levels of management to carry out project related tasks of various size and scope
Exemplify a commitment to THP’s vision, mission and values and demonstrates dedication to a patient-centered approach to delivering quality hospital care
We are looking for a strong team player with interests and/or experience in strategic planning, project management, change management, and business case development. We are seeking candidates who can balance priorities and perform in fast-paced, changing environments. Most importantly we are looking for candidates with a passion for health care and who share our values of excellence, compassion, and courage.
