Learn more at shn.ca Job Description: Position Overview: Reporting to the Manager of Medicine the successful candidate will be responsible for overall quality of service, coordination of patient and family care, coordination of staffing, effective & efficient resource utilization, facilitating relevant patient access and flow, maintaining data accountability and acting as a steward for developing and facilitating the family-centered care approach within the Medicine clinic. The candidate will also be responsible for assessing, planning and implementing plans of care with the physicians. Requirements: • Current certification with the College of Nurses of Ontario • Competent to perform CPR at a Basic Cardiac Life Support level, current BCLS • Minimum 5 years recent Acute Care In-Patient Medicine and or Outpatient Medical/ Surgical experience • Minimum of 2 - 3 years of recent Charge Nurse experience in In-Patient Medicine and/or outpatient clinics • Demonstrated leadership, coordinating skills, teaching and technical ability • Excellent communication, documentation, and interpersonal skills with the ability to work well with patients, families, physicians, and staff • Ability to work well independently and to demonstrate well developed critical thinking skills • Ability to advocate for quality-based patient outcomes within an interdisciplinary team environment • Exposure to Lean principles, an understanding of methodologies and tools and a commitment to the use of these in support of providing the best healthcare experience possible to our patients • Demonstrates an understanding of and commitment to SHN’s comprehensive safety programs and practices including staff, patient, and environmental safety • Evidence of sound organizational /prioritization skills, including an energetic, confident, and resourceful attitude, with the motivation to deliver results in a complex and demanding environment • Experience and interpersonal skill set that facilitates provision of consultation, education and creative, enhanced problem solving to the care team, (including family, formal and informal care providers). • Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers, other members of the community and public • Demonstrated excellent problem-solving, decision making and critical thinking skills • Demonstrated excellent organizational, time-management and team building skills • Demonstrated initiative and the ability to function both independently and as a multidisciplinary team member in a fast-paced demanding environment • Demonstrates values consistent with SHN's Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation. • Utilize protective equipment provided and identify and reports hazards. • Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community. • Excellent pattern of good attendance and work record Asset: •BScN •Chronic diseases management experience •Experience in quality improvement processes •Ability to evaluate patient and family outcomes, facilitate modification of care plans as required to promote optimized function and quality of life •Ability to speak another language Accommodation and Diversity Statement: Scarborough Health Network (SHN) embraces and celebrates our community’s unique multicultural heritage and diversity.
