Position Summary:
The Community Navigator acts as the initial contact for jobseekers and community partners. This role develops and maintains referral pathways, facilitates warm handoffs to Service Providers, and ensures jobseekers receive suitable services. The Community Navigator works closely with internal teams and external stakeholders to track referrals and manage risks. It is a hands-on position that requires strong relationship management, excellent organizational skills, and a commitment to providing equitable employment services.
What You'll Do:
Respond promptly to referral requests via phone and email, same day, max 2 days
Ensure contract compliance for all Request a Service Provider Appointment (RASP) and Targeting, Referrals and Feedback (TRF) inquiries
Ensure contract compliance for all Ontario Works jobseeker referrals
Establish rapport with jobseekers and conduct warm handoffs to Service Providers.
Follow up with jobseekers to ensure services are received.
Track activity using Monday.com, MS Excel, and FedcapCARES.
Prepare reports on referral activity and outcomes.
Respond to all Service Provider-related issues and complaints within two business days, with resolution or escalation within 5 days.
Compliance & Risk Management
Ensure 90% compliance for all Request a Service Provider Appointment (RASP) and Targeting, Referrals and Feedback (TRF) inquiries
Ensure 90% compliance for all Ontario Works jobseeker referrals
Convert 60 % of inquiries to booked appointments with Service Providers
Respond to all Service Provider-related issues and complaints within two business days, with resolution or escalation within 5 days.
Administrative & Data Management
Maintain an internal activity tracker with 100% accuracy, logging jobseeker intake appointments, follow-ups, and outcomes.
Collaboration & Stakeholder Engagement
Represent Fedcap Canada at community events
Participate in monthly alignment meetings with internal Fedcap teams to ensure consistent communication and priorities across catchments.
You're a Great Fit for This Role If You Have:
Diploma or Degree in Social Services, Public Administration, Public Health, Marketing, Human Resources, or related field.
Experience working with community-based organizations government entities or employment service networks.
Experience working with individuals with barriers to employment.
Knowledge of economic and workforce development and community health a plus.
Compensation:
$50,000-$65,000 CAD
