e4c’s Youth Housing Programs (YHP) offers Community Group Care that journeys with youth aged 13 to 17 years old that have complex needs and require trauma-informed, developmentally responsive, therapeutic milieu care environments that can safely and successfully meet their care needs. As a Lead Youth Worker, you will play a key role in ensuring the safety, development, and overall well-being of youth through individualized care, relational practice, and effective case management. You will work collaboratively with each youth’s Care Team to support their spiritual, physical, emotional, and mental wellness, while fostering life-skills development, promoting wellness, and encouraging meaningful family involvement. In this leadership role, you will also ensure the delivery of high-quality case management and care planning to support each youth’s successful transition into young adulthood.
Key Responsibilities:
Case Management
- Lead youth intake assessments, case planning, and transition strategies
- Support youth with education, life skills, employment, and goal completion
- Coordinate services and referrals with legal, medical, and social agencies
- Review and maintain youth files, incident reports, and daily notes
- Facilitate communication with youth guardians and community supports
Team & Program Leadership
- Provide leadership in scheduling, documentation oversight, and team meetings
- Fill shift gaps and ensure house coverage aligns with program needs
- Mentor Youth Workers and ensure consistency in youth engagement practices
- Oversee house budgets and daily operations in partnership with management
Youth Engagement & Support
- Build rapport with youth using a trauma-informed, harm reduction approach
- Provide crisis intervention, de-escalation, and one-on-one support
- Promote skill-building, conflict resolution, and positive communication
- Support recreation, community involvement, and youth-led activities
Community & Compliance
- Foster positive relationships with neighbors and local services
- Ensure compliance with all legislation and standards (OHS, CARF, etc.)
- Document and follow up on community incidents or concerns
What you bring to the team:
- Diploma in Child and Youth Care (CYC), Social Work, Human Services, or related field
- 3+ years supporting marginalized youth or vulnerable populations
- 1+ year experience leading case files or care planning
- Strong crisis response, harm reduction, and trauma-informed skills
- Comfortable working independently and in a team
- Proficient with Microsoft Office and case management tools
- A valid driver’s license and access to a reliable vehicle are required

Schedule:
- Monday to Friday (rotation: 7am to 3:30pm and 7:30am to 4pm)
- Participation in on-call coverage rotation.
