The Geriatric Nurse receives and triages referrals and liaises between the referring team, patient/family and the Geriatrician/Resident to provide fulsome clinical background and assessment data. They contribute to the overall comprehensive geriatric assessment and assist referring teams to implement recommendations. They also provide mentorship and support to referring nursing team members regarding geriatric syndromes (e.g. – Dementia, Delirium, Multiple Co-Morbidities, Frailty, Falls Risk, Medication concerns, etc.). They facilitate the screening and approval process with referring teams and Geriatricians for all referrals to the GRU. They collaborate with the GRU Team to maintain and prioritize the GRU Waiting List and coordinate with them regarding admission planning. Travel between sites is required.
- Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse
- Baccalaureate Degree in Nursing is preferred
- Current Certificate in Basic Cardiac Life Support (B.C.L.S.)
- A minimum of 5 years of relevant gerontological nursing experience within the last 7 years
- Completion of additional post-secondary courses in gerontology
- Demonstrated ability to apply gerontology/rehabilitation nursing knowledge in the care of older adults
- Demonstrated ability to provide consultation services to acute care units, and develop ongoing collaborative
relationships with acute care, community agencies and physicians
- Demonstrated excellence in developing interpersonal relationships, communication and problem-solving skills and
managing competing priorities within a rapidly changing healthcare environment
- Demonstrated ability to work both autonomously and within an interdisciplinary team environment
- Demonstrated ability to facilitate the interrelationship between older adults/families and the health care system,
Including a knowledge of local community resources
- Membership in professional organizations, e.g., provincial and/or national gerontological nursing organizations
- Participates in evaluation and research initiatives including CQI process improvement projects
- Demonstrated willingness to learn and commit to personal growth through ongoing education
- Proficiency with Microsoft Office, Meditech, Clinical Connect
