The Director of Clinical Programs - Behavioural Supports, Falls PASD, Restraints will be reporting to the Assistant Vice President of Clinical Services.
- Highly competent \& high functioning Village Dementia/Personal Expression and Falls, PASD \& Restraint Leads
- Support and maintain the Living in My Today Philosophy of Care
- Build and maintain competency and capacity of Dementia/Personal Expression and Falls, PASD \& Restraint Leads and Regional Clinical Specialists through Village visits across the province
- Build culture of compliance and continuous improvement of Dementia/Personal Expression and Living in My Today dimensions by monitoring and improving quality of care provided to residents in LTC and RH
Key Responsibilities: The Director of Clinical Programs supports the Responsive Behaviours, Falls, PASDs and Restraints programs for the organization across our Long Term Care and Retirement Homes. Training \& Education
- To develop, educate, onboard, coach, mentor and support Village Clinical Leaders (across all Villages through the province) and Team Members through consultation with Village Leadership Team, SVP/VP Operations and AVP Clinical Support as applicable
- To support the professional practice of the Village Leadership Team as applicable
- To consistently deliver education with Schlegel Villages programs/initiatives/policies \& procedures
- To assist in the implementation of new programs and in the revision and update of programs
- To provide innovative suggestions to assist with overall implementation of new concepts in order to minimize potential negative perception of change
Support \& Data Analytics
- Work in partnership with our Director of Dementia Services \& Knowledge Integration
- Collaborate and support the PAL (Program for Active Living), Recreation, Nutrition \& Food Service and People Teams
- Assess, analyze and implement protocols and practices in compliance with provincial and best practice guidelines
- To ensure follow up of high-risk issues
- To support Villages in meeting regulatory requirements
- To uphold \& promote Schlegel Village Culture and Values
- To promote compliance with regulatory requirements and Schlegel Village standards
- Facilitate CoP (Community of Practice Meetings) with all Dementia/Personal Expression and Falls, PASD's \& Restraints Leads on a regular basis
Continuous Quality Improvement
- Lead and support CQI/QIP on reduction of antipsychotic use, falls, PASD and restraints
- Act as a liaison and subject matter expert to all external and internal stakeholders
- To ensure the effective operation and Continuous Quality Improvement (CQI) of the homes
- To ensure CQI \& Accreditation programs are managed in accordance to Schlegel Village standards, policy \& practices, and in compliance with legislative and regulatory requirements
- To support utilization of the quality improvement framework on clinical programs through program review, audits and data analysis
- Develop and ongoing review of Dementia Manual (Policies, Procedures, Education \& Tools) to ensure meet required standards
- To monitor, analyze and support quality indicators and adverse events
- To conduct quality reviews based on outcomes as supported by AVP Clinical Support
Required Education, Credentials \& Experience
- Registered Nursing Team Member (CNO certification) \& Current registration and in good standing with a regulatory body in Ontario
- Previous experience with Dementia, Responsive Behaviors, BSO, Personal Expressions
- Knowledge and/or job related experience in health care practices
- Proficiency in the use of computer databases, spreadsheets and literature search
- Demonstrated ability to work collaboratively in a multidisciplinary team environment
- Self-directed, well-organized and detail-oriented
- Excellent interpersonal, communication and presentation skills
- Ability to run reports and trend and analyze data on Expressions/Responsive Behaviours, Falls, PASD \& Restraints
