The Resident \& Family Experience Coordinator III plays a critical role within our communities, impacting residents' lives and fostering human connection.
As a Resident \& Family Experience Coordinator III, your typical day will include:
- Leading the resident move-in, transfer, and move-out process in line with organizational standards, including facilitating applications, assisting with financial supports, conducting tours, and introducing residents/families/SDMs to the community and its operations.
- Supporting residents and families through social work counselling, collaborating with the inter-professional team to assess and develop care plans that address psychosocial and social needs, promoting overall well-being and safety.
- Planning, developing, implementing, and evaluating social work services and support groups tailored to residents' and families' needs, engaging external resources as necessary.
- Assisting residents and families with advanced care planning and treatment discussions within the scope of Social Services, including funeral arrangements.
- Recruiting and supervising social work students' placements.
- Managing feedback received in the resident home area/neighborhood, ensuring timely responses and satisfactory outcomes.
- Providing reports and analysis of feedback to Leadership \& Quality Committees.
Must haves:
- Registered Social Worker as per provincial legislation.
- Minimum of three years' experience in healthcare, social services, or related sectors.
- Knowledge of the Long Term Care Act \& Regulations (preferred).
- Customer service experience (asset).
- Hospitality and/or sales experience (asset).
- Strong oral and written communication skills.
- Organizational, planning, and delegation abilities.
