POSITION: Adult Complex Care Case Manager (Social Services)
LOCATION: Barrie, with responsibility throughout Simcoe County
This is a unionized position.
PURPOSE OF POSITION
The Adult Complex Care Case Manager provides a specialized case management function to implement a Ministry of Children and Community Social Services funded service plan for adults who reside in a supported living environment on a fee for services basis, or who reside in their own or their family home and purchase the services they require to meet their needs. The Adult Complex Care Case Manager provides ongoing oversight and evaluation to ensure that the plan continues to be relevant, and in the individual’s best interest. The Adult Complex Care Case Manager ensures that the funds are being utilized appropriately and according to their service plan. The Adult Complex Care Case Manager also provides support and referrals to access a range of required services.
STATEMENT OF MAJOR RESPONSIBILITIES
To assist with the development, implementation, and monitoring of the approved integrated service In so doing, the case manager:
Meets with all relevant parties (including the individual, family, community providers) to ensure that everyone agrees with the plan and gathers information and plan for implementation
Researches the availability and suitability of supporting living placement resources if these are specified in the plan
Researches the availability and suitability of in-home or in-community resources if these are specified in the plan
Develop a budget for implementation of the plan
Work with the family and community and/or supported living resources to plan or the transition
Ensure that all aspects of the plan are implemented as intended and in as seamless a manner as possible
To ensure that the plan meets (and continues over time to meet) the needs of the individual and, where appropriate, the In so doing, the Case Manager:
Formulates a current understanding of the individual’s needs, respecting values, preferences, beliefs, heritage, and identity on an ongoing basis
Arranges for and/or facilitates the provision of specialized assessments as needed
Participates in the development and monitoring of the plan of care in supported living settings in accordance with legislative requirements
Refers and assists in accessing required services
Provides social advocacy support
Monitor delivery of contracted services identified in the plan qualitatively and monetarily
Evaluates the plan to ensure ongoing appropriateness to meet individuals
Initiates planning for significant transitions and works to make these transitions as seamless as possible
To maintain an organized and accurate paper/electronic clinical record that demonstrates the provision of services, establishes accountability, and enables the evaluation of service quality for all persons served that includes:
Recording and reporting client information necessary for effective case management and preparing progress reports
Collecting and reporting service delivery data as required
Adhering to all agency standards and guidelines and procedures
To promote the agency through presentations, literature, and inter-agency functions
To attend and participate in supervision, team meetings, and performance appraisal
To participate in professional development in consultation with manager and demonstrate initiative in learning and developing new techniques of service
To practice effective communication with co-workers, consulting and sharing resource information and providing case coverage as required
To participate in committee, work internally or externally as assigned
To adhere to all agency standards and guidelines and procedures including health and safety
To carry out such other duties as may be assigned by the Supervisor that are consistent with the incumbent’s position
QUALIFICATIONS:
Education and Experience Level
Post-secondary degree at a university level in one of the social or behavioral disciplines with directly related experience (3 to 5 years) in working with children/youth and adults with multiple and complex areas of exceptionality (developmental disability, emotional/psychiatric needs, medically fragile, acquired brain injury) and their families
Knowledge and Skills
Knowledge of normal and abnormal development.
Knowledge of family systems theory and an understanding of the family development and functioning.
Ability to develop trusting, co-operative relationships with clients, families, service providers, and other stakeholders.
Ability to demonstrate sound clinical judgement with respect to complex client situations
Excellent communication and negotiating skills and ability to manage communication.
Computer/keyboarding skills are required.
Knowledge of community resources for youth and adults with special needs and relevant legislation.
Ability to support self-advocacy and empowerment for families.
Sensitivity, understanding and ability to work with a variety of value systems, beliefs and cultures.
Working knowledge of word processing and data entry
Capacity to work independently
Aware of Health & Safety Rules & Regulations.
A working knowledge of French Language is beneficial
Valid driver’s license and access to a reliable vehicle.
Hourly Rate Range: $27.78-$31.48 per hour, plus a generous employee Benefit package, including Wellness Program, Employee Assistance Program, Pension Plan & Health and Dental Plan.
