Location & Schedule:
In Person position in Little Canada, MN
Office Hours - Monday to Friday, 8:30am - 5:00pm
Primary Responsibilities:
Act as first point of contact for Advanced Medical Home Care by providing excellent customer service to medical partners, patients, and referral sources. Review referral documentation and create new patient charts with complete and accurate medical data in electronic medical records. Obtain valid face-to-face documentation from physicians and upload complete documentation to EMR. Determine insurance eligibility of new patients. Assist in managing patient hospitalizations and reporting patient status to appropriate departments.
Specific Job Duties – include, but are not limited to:
Answer referral calls, patient and provider inquiries over the phone
Create patient charts accurately and completely
Determine insurance eligibility
Welcome new patients via telephone and schedule their initial visits
Help manage intake referral email and faxes
Assist in managing patient hospitalizations
Upload completed documentation to EMR system
Obtain valid face-to-face documentation from physicians
Request and receive verbal orders from providers
All other duties and tasks as assigned

Qualifications
Experience:
2 years of medical office experience and/or direct patient care.
Ability to provide excellent customer service
Demonstrated ability to multi-task and manage time
Ability to work and communicate professionally with patients, medical partners, employees, and supervisors
Ability to work independently and as part of a team
Required:
Current Minnesota PTA/LPN/COTA license in good standing
Knowledge of medical terminology
Excellent communication skills
Good judgment and decision-making skills
Strong attention to detail
Technologically proficient and driven by accuracy

Regular attendance and flexibility with scheduling necessary
