In this role you will be responsible for conducting assessments, developing and delivering treatment plans focused on achieving the goals of care including mobility and function, symptom management, and delivering education.Work under the supervision of the Program DirectorCollaborate with the interdisciplinary team to create personalized care plans based on the patient’s unique needs and goals of careParticipate in clinical huddles and case conferences with team members such as physicians, nurses, social workers, speech therapists and occupational therapistsConduct physiotherapy assessment, consultation and planned interventions for patientsEngage in skilled, compassionate communication with patients, families, and caregivers regarding mobility, and functionEducate and mentors patients, family and caregivers on strategies to maintain safe mobility and function in the home settingMaintain accurate and timely documentation of patient assessments, treatment plans, and outcomes, ensuring adherence to ethical and legal standardsQualificationsMaster’s degree in PhysiotherapyRegistered in good standing with the College of Physiotherapists of Ontario3-5 years of experience working as a Physiotherapist preferredValid Ontario driver's license and a reliable vehicleAbility to work autonomously and as part of an integrated health care team, with an emphasis on collaborative care planningProficiency in conducting assessments, interpreting findings, and formulating evidence-based treatment plansFamiliarity with assistive devices, adaptive equipment, and environmental modifications#palpeel_2
