Join our team today!Job DescriptionAccountabilities:Work collaboratively with the Pharmacy Team to provide patient-centred careDispensary duties including medication order entry and medication profile reviewProvide drug information to physicians, nurses, and other clinicians to enhance patient careVerification of all medication orders for inpatientsProvide therapeutic drug monitoringParticipate on hospital committees as requiredPerform and/or supervise in patient dispensing (which includes computerized order entry, unit dose, centralized IV admixture services, TPN, Chemotherapy and an understanding of automated systems) drug information and administrative activities as assignedResolve day-to-day clinical issuesAssist with the implementation of pharmacy initiatives, which improve patient care and other initiatives which improve the efficiency and effectiveness of the Pharmacy operationsOther duties as assignedQualificationsCurrently licensed (or eligible) by the Ontario College of Pharmacists, Part A of the registryRelevant clinical experience preferableExcellent attendance, punctuality and work recordMay be required to travel between and work at all Osler sitesExcellent computer skillsExcellent oral, interpersonal, and written skillsDemonstrate initiative and a willingness to work collaboratively in a team environmentStrong clinical assessment skills in identifying and resolving drug-related problemsDemonstrated ability to apply clinical judgment in analyzing and solving clinical problemsMeditech computer experience preferableMust be able to meet the physical demands of the positionMust demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation and CollaborationAdditional InformationHours: Nights, including weekends and statutory holidays (subject to change in accordance with operational requirements)SALARY:
