We are opening the doors to a state-of-the-art treatment hub designed to provide comprehensive support and resources that address the root causes of homelessness and addiction. We are currently seeking a 1 FTE Nurse Practitioner who will be responsible for working with an interdisciplinary team to provide a full primary care scope of practice to clients of the HART Hub. There is a strong practice emphasis on disease prevention, health promotion, health education, and community partnerships.
One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.
Eligible for the Community Commitment Program for Nurses (CCPN) that provides $25,000 in grant funding to eligible nurses in exchange for a 2 year commitment.
The CCPN is now offering two new Northern incentives for nurses who make a two-year commitment to an eligible employer in Northern Ontario. These are:
A Northern Top-Up Grant of $10,000
A Northern Relocation Grant of $10,000

QUALIFICATIONS:
BScN in nursing from a recognized University and in addition a certificate in the extended class from the College of Nurses of Ontario (CNO).
Thorough knowledge and proficiency in current extended class nursing procedures, physical assessment and treatment measures within the RN-EC scope of practice.
Experience in a community-based primary health care setting, addictions, homelessness, and/or mental health is an asset.
Strong understanding and alignment with the concepts of trauma-informed care and culturally safer care.
Ability to navigate complex case management situations, advocate for clients effectively, and create individualized care plans.
