Job Description Position Summary: The Skin and Wound Care Nurse plays a vital role in promoting skin integrity and managing complex wounds for residents in our long-term care facility. This part-time position focuses on assessment, treatment planning, staff education, and quality improvement related to skin and wound care. Key Responsibilities: Conduct comprehensive skin assessments for residents, including pressure injuries, surgical wounds, diabetic ulcers, and other skin conditions. Develop and implement individualized wound care plans in collaboration with the interdisciplinary team. Monitor and evaluate wound healing progress; revise care plans as needed. Provide clinical expertise and guidance to nursing staff on best practices for skin integrity and wound care. Facilitate education sessions and hands-on training for staff on wound prevention, dressing techniques, and infection control. Maintain accurate and timely documentation in resident health records and other reporting tools. Participate in quality improvement initiatives related to wound care and infection prevention. Collaborate with physicians, pharmacists, and allied health professionals to ensure continuity of care. Ensure compliance with all applicable standards, policies, and regulations. Qualifications: Current registration with the College of Nurses of Ontario (CNO) in good standing. SWAN program completed or enrolled desirable In-depth knowledge of wound prevention and management, skin integrity, and pressure injury protocols. Strong assessment, communication, and teaching skills. Ability to work independently and collaboratively in a team environment.
