SUMMARY OF DUTIES
Reports to the Registered Nurse (RN). Provides nursing care in accordance with physician orders and Standards of Nursing Practice.
GENERAL DUTIES
Administer and record prescribed medications and treatments to residents. Actively participate in processing medication and treatment orders in accordance with established Lodge policies.
Clean and stock treatment and medication carts.
Provide total personal care such as bathing, dressing, grooming, toileting and feeding to able and disabled residents.
Ensure residents have an individualized plan of care, documented with the use of the electronic software, as per mandate from the Ministry of Long-Term Care.
Audit and complete documentation for residents on a daily, quarterly, annual and as needed basis.
As assigned and needed, participate fully in the ongoing assessment of residents using the RAI-MDS.
Assist able and disabled residents to in-Lodge appointments, meals and activities.
Record personal care given to residents; observe and report symptoms, conditions and behaviour.
Take residents’ vital signs.
Receive and answer inquiries from staff, other departments, residents and families.
Monitor residents; report accidents.
Assist residents to wheelchair from bed or bath with minimal physical assistance, or with the use of a mechanical lift, as appropriate.
Collect and label specimens for resident check-ups.
Distribute meals and nourishment.
Respond to emergency situations involving residents. Without direction is able to initiate assessment and treatment, and then consult with RNs as needed to ensure residents receive appropriate post incident care.
In collaboration with the RNs will consult with other members of the interdisciplinary team to maximize care and health opportunities for assigned residents.
Respond to emergency evacuations by transporting able and disabled persons.
In collaboration with RNs provide clinical direction to our non-regulated health professionals (Personal Support Workers - PSWs).
Work in accordance with the provisions of applicable Health and Safety legislation and all City of Hamilton corporate and departmental policies and procedures related to Occupational Health and Safety.
Perform other duties as assigned which are directly related to the major responsibilities of the job.
QUALIFICATIONS
Practical knowledge and skills related to the nursing field acquired through the graduation from a recognized Practical Nursing program.
Demonstrated ability to administer and record prescribed medication, take vital signs and observe symptoms, conditions and behaviour of residents.
Current Registered Practical Nurses (RPN) registration with the College of Nurses of Ontario. Sound knowledge and demonstration of practice that is based on the Standards of Practice that govern and direct nurses in Ontario.
Completion of the Administration of Medication for Registered Practical Nurses.
Effective communication skills and demonstrated ability to relate to peers and seniors in a Home for the Aged.
Proficient computer skills and knowledge related to ‘Point Click Care’.
Proficient with RAI MDS (Coding, RAPS & RUGS).
Proficient with eMAR (electronic medication administration record).
Excellent written skills.
Excellent customer service skills for both internal and external customers.
Positive professional image.
Excellent assessment and care planning skills.
Working knowledge of the legislation and standards that govern LTC Homes in Ontario. Experience working with High Intensity Needs clients and the High Intensity Needs Funding process.
Experience in long term care preferred.
