Program Description:
The Emergency Treatment Fund (ETF) is a funding initiative offered by Health Canada to municipalities and Indigenous communities to address urgent substance use needs.
The HOME Team is the Guelph Community Health Centre’s contribution to ETF along with the Sanguen Health Centre, and the Mount Forest Family Health Team, which will provide mobile wrap-around support services to individuals struggling with substance use who face barriers to accessing traditional services. The HOME team will expand on existing programs to operate a mobile outreach vehicle in the City of Guelph, in tandem with Sanguen’s Mobile Health Van in Wellington County, and offer additional place-based supports.
Reporting to the Guelph CHC Allied Health Supervisor, the HOME Outreach Worker will work in tandem with a HOME Peer to seek out and engage with community members facing barriers to accessing service, particularly due to being unhoused and marginalized. By building rapport with clients, they will seek to reduce barriers to access and increase client interaction with social and health services.
PRINCIPAL JOB DUTIES
To meet clients where they are at in terms of motivation, need, and geography.
Build rapport with clients to gain trust and engagement that can be leveraged to deepen their connection to relevant services
Support access to food, housing, financial assistance, as well as mental health and substance use services
Document client interactions, as needed
Receive and support referrals from clients, service organizations, and relevant institutions
Coordinate with other outreach-based providers and organizations to provide responsive, community-based care safely
The ability to lift, move or manipulate objects up to 50 lbs
Manually load and unload materials from pallets, skids, platforms, cars, and lifting devices
Other projects related to ETF, as needed
KNOWLEDGE, SKILLS & ABILITIES
Post-secondary degree or diploma in social service-related discipline.
Emergency First Aid and CPR Certification Level C – Mandatory.
Driver’s license and insurance – Mandatory
Lived experience with substance use and/or mental health an asset
Experience working with individuals marginalized by their mental and/or substance use health, alienated from services, and frequently in crisis, both in-patient and community settings.
Demonstrated knowledge of community resources related to the served population in Guelph.
Awareness of and comfort attending community locations such as social service offices, community food pantries, drop-in centres, and outdoor spaces frequented by relevant clientele
Ability to work independently, in the community, and within an interdisciplinary team environment.
Engaging, collaborative and creative, astute to clients' needs with the ability to adapt and be flexible.
Excellent assessment, communication and organizational skills.
