Program Description:
The Emergency Treatment Fund (ETF) is a funding initiative offered by Health Canada to municipalities and Indigenous communities to address urgent substance use needs.
The HOME (Health Outreach Mobile Engagement)
Team is the Guelph Community Health Centre’s contribution to ETF along with the Sanguen Health Centre, and the Mount Forest Family Health Team, which will provide mobile wrap-around support services to individuals struggling with substance use who face barriers to accessing traditional services. The HOME team will expand on existing programs to operate a mobile outreach vehicle in the City of Guelph, in tandem with Sanguen’s Mobile Health Van in Wellington County, and offer additional place-based supports.
Reporting to the Guelph CHC Allied Health Supervisor, the HOME Team Counsellor will provide substance use counselling and generalized counselling support to community members engaged with the HOME Team in Guelph and Sanguen/Mount Forest FHT in the County of Wellington. They will work with clients where they are at to support the creation and pursuit of health-oriented goals through the development of a therapeutic relationship.
PRINCIPAL JOB DUTIES
Meaningfully engage and build authentic relationships with individuals who may struggle to trust service providers.
Maximize wellbeing and independence of clients by providing psychosocial counselling using a trauma informed lens
Provide crisis, short-term, and ongoing care to clients using a variety of approaches counseling techniques including motivational approaches, first stage trauma treatment, CBT and DBT skills
Develop stage-matched, client-centred care plans
Collaborate effectively as part of an integrated, multi-disciplinary team
Collaborate and work effectively with community partners in the delivery of care for individuals with complex needs
Complete documentation, reporting, and administrative responsibilities as required
Support quality improvement initiatives at the program and organizational level as required
Work effectively with individuals at all points on the harm reduction continuum
KNOWLEDGE, SKILLS & ABILITIES
Registered health professional with knowledge and experience in the provision of counselling/psychotherapy preferred
3-5 years of experience working with individuals who use substances in a community-based setting
Practice from a strengths-based,
anti-oppressive, and trauma informed perspective
Knowledge of, and experience with, first stage trauma treatment (safety/stabilization/knowledge building), holistic assessment, and system-based thinking.
Thorough knowledge and understanding of community resources, preferably Guelph and Wellington County resources.
Ability to multitask and work independently
Effective written and verbal communication skills
Effective time management, decision making and organizational skills
A good team member and supportive of interdisciplinary practice.
