Job Description: Under the Program Manager's direction and as part of the interdisciplinary team, the Assessor Coordinator provides screening, triage, assessment, treatment and case coordination services to a community mental health and addictions population. The Community Outreach & Support Team provides crisis intervention, support, assessment and follow-up plans to clients. The team assists in stabilization and/or transition of acute patients from hospital/detox to community or alternatively to prevent admission/readmission by providing short term acute mental health/addiction services to those in crisis. Follow-up may include brief monitoring/support, referral to appropriate community organizations/support networks or collaboration with supportive organizations. Services are provided through extended hours (days, evenings and weekends).

Education
Recognized degree/diploma in nursing; degree in social work; or a degree in addiction counselling.
Experience
One year (postgraduate) recognized experience in working within community mental health and addiction services.
Knowledge, Skills and Abilities
Knowledge of a variety of mental health disorders and addiction and harm reduction strategies required to enable clients to sustain stability.
Knowledge of community resources including treatment programs and support agencies is essential.
Experience working with mental health and addiction community outreach and competence in delivering mental health and addiction crisis interventions.
Knowledge of a variety of intervention strategies (individual and group) for individuals with mental health and addiction problems in a community based setting, including, but not limited to: assessment, triage, treatment, case management.
Ability to identify, implement and evaluate areas of client difficulty across a wide range of clinical presentation and formulate evidence based intervention.
Cultural competency skills.
Knowledge of Recovery philosophy.
Ability to work independently and in a self-motivated manner, ability to respond under pressure.
Ability to work in a multidisciplinary setting.
Knowledge and practical application of the Mental Health Services Act and Health Information Protection Act.
Public relation skills with the ability to maintain effective work relationships.
Interpersonal competence including effective problem solving; workload management and conflict resolution skills.
Excellent communication skills (written and verbal). Ability to prepare reports in a clear concise and timely manner. Ability to provide public presentations.
Demonstrated ability to enter and retrieve data in a computer database system.
Other Information
Must have a current practicing registration with their professional association.
Must have a valid driver’s license. Some assignments may require access to a vehicle for work.
Basic computer skills.
Ability to work flexible hours; including an ability to work day, evening and weekend shifts based on operational needs.
