Overview
SEOCHC’s Preventative Care & Wellness Clinics for Priority Populations is an innovative program designed to provide accessible, equitable and high-quality care to our community. At the heart of our mission is a commitment to servicing vulnerable populations, including newcomers, members of the African, Caribbean and Black (ACB) community, LGBTQIA+ individuals and other underserved groups. Our Walk-in Clinic will provide locally informed, culturally responsive interventions and clinical care to advance access to healthcare services with a focus on health
promotion, preventative care and culturally responsive prevention models including proven cancer and chronic disease screening models. Our clinic offers walk-in and virtual care services 5 days/week, including evenings and weekends.
The RN will work collaboratively with an interprofessional team, including NPs, RMs, LCs, social workers and administrative assistants, to provide education, screening, chronic disease prevention and wellness support. Given that this is a new program, the RN is expected to actively contribute to its development, including refining workflows, identifying service gaps, and ensuring continuous improvement in service delivery.
Key Responsibilities
Program Development & Implementation
Play a role in shaping the structure and processes of the clinic
Identify and implement best practices for preventative care services tailored to priority populations
Work with leadership to assess community needs and adjust clinic services accordingly
Clinical Care & Health Promotion
Perform comprehensive health assessments, including gathering health histories, performing physical exams and checking vital signs
Triage walk-in clients to prioritize care based on urgency and refer them to appropriate providers when necessary
Provide treatment within the RN’s scope of practice such as administering medications, immunizations, wound care, POCT etc.
Deliver health education and promotion tailored to the unique needs of clients with a focus on prevention and wellness
Screening and education of chronic disease management
Offer culturally responsive, trauma-informed, and harm-reduction care tailored to diverse community needs
Complete timely and accurate medical records of client encounters and documentation
Utilize evidence-informed approaches to promote health equity and disease prevention
Support clients in navigating healthcare system and accessing appropriate resources
Advocate for and connect vulnerable populations to community supports
Participation in pop-up wellness clinics in the community
Conduct client education sessions on topics such as diabetes management, smoking cessation, sexual health, nutrition etc.
Implement and enforce PPE practices
Collaboration & Teamwork
Work in partnership with interprofessional team to provide holistic, client-centred and integrated care
Support the NP with referrals to specialists and other disciplines such as PT, OT, SLP etc. as needed
Support the NP in completing forms for social supports, such as ODSP, WSIB, ADP etc. as needed.
Engage with community partners to enhance service offerings and client support
Participation in mandatory team meetings, education and training as assigned
Work with the team to ensure clinic rooms are organized and well stocked
Take lead role re: supplies and equipment
Data Collection & Quality Improvement
Identify service gaps and contribute to continuous quality improvement efforts
Support the development of protocols, procedures and evaluation measures for the clinic
Other duties as assigned
Qualifications
Bachelor’s degree in nursing
Registration in good standing with the CNO as an RN
At least 3 years of experience working as in primary care, serving equity-deserving members of the community
Strong critical thinking, problem-solving skills, organizational and interpersonal skills with an ability to work effectively within a multidisciplinary team environment
Ability to thrive in a busy and fast-paced environment
Strong understanding of social determinants of health and their impact on health outcomes
Familiarity with trauma-informed, culturally responsive and anti-oppressive care practices
Strong communication, interpersonal and conflict resolution skills
Proficiency with EMR systems
Vulnerable sector police check completed with the last 12 months
Proof of COVID-19 vaccination status will be required for employment
Languages spoken other than English and French
Salary
$72,290 - $85,048 (commensurate with experience)
