This Therapist will cross cover 5C Rehab, UHN RCC, Humber RCC, Moir CCC, McCall Centre and potentially other areas of the transition and rehab services.

Job Description:
The Physiotherapist is accountable for the following:
Assessment of patients with physical dysfunction of the cardio-pulmonary, musculo-skeletal and neurological systems and co-creation of goals with the patient and family
Planning and implementation of treatment plans to improve movement/function, alleviate pain, and optimize respiratory function
Participate in patient discharge planning
Document patient assessment and treatment in the health record
Collaboration with the interprofessional healthcare team, patients and their families to achieve patient goals and augment the treatment plan as needed
Providing education to the interprofessional team to support evidence-based care
Assigning to and supervision of support personnel
Involvement in clinical education of various professions, including preceptorship of physical therapy students
Key Qualifications, Skills and Abilities :
Registration with the College of Physiotherapists of Ontario
C.P.A. member preferred
A breadth of experience is required, including but not limited to assessment and treatment of post-acute medically complex, orthopaedic, neurogenic, geriatric and vascular patients (including amputees)
Experience with acute medicine patients and transitions in care is an asset
Knowledge of community resources and services is an asset
Rostered with the College of Physiotherapists of Ontario for oxygen titration or willingness to complete process within 3 months of hire
Certified in FIM assessment, or willingness to be certified within 6 months of hire
Superior interpersonal and communication skills
Demonstrated effectiveness working independently as well as liaising with the interprofessional team
Current certification in Basic Cardiac Life Support preferred
Demonstrated leadership within a interprofessional team is preferred
Satisfactory performance and attendance required
Position Summary:
As an active member of a multidisciplinary team, the dual trained OTA/PTA provides client-centered clinical care for assigned patients under the direction and supervision of a Physiotherapist and Occupational Therapist.
Provides goal-oriented treatment interventions, patient/family education and support to address physical, emotional and self-care needs as assigned
Provides timely and astute information to relevant team members about patient’s performance
The ability to perform as a coach/mentor and to use sound critical thinking skills is key to achieving patient outcomes and to facilitating team function
Participates in program planning, continuous quality improvement and evaluation
Supervise activities of OTA/PTA students, volunteers and co-op students as required
