As a member of the healthcare team on M3, the Registered Practical Nurse assesses and provides nursing care to meet the needs of mostly senior patients suffering from a variety of complex medical health conditions and works collaboratively with Registered Nurses and other members of the interdisciplinary healthcare team to optimize the functional goals of patients. May be required to provide coverage to other clinical settings within HHS as needed.
- Current Certificate of Registration with the College of Nursing of Ontario as a Registered Practical Nurse
- Demonstrated knowledge of Rehabilitation nursing
- Knowledge of outcome measurement, eg. Functional Independence Measure
- Familiar with a Primary Nursing model of care
- Demonstrated commitment to interdisciplinary care and a nursing professional practice model
- Current BCLS certification
- Recent clinical experience (within 2 years) with a rehabilitation, medical, or geriatric patient population
- Experience and skill in pain management, providing enteral and parenteral nutrition, complex wound care, and vascular access.
- Successful completion of the Gentle Persuasive Approach (GPA) workshop
- Demonstrated excellent communication (written and verbal), teamwork, problem-solving, time management, organization, and interpersonal skills
- Demonstrated computer skills including Meditech
- Demonstrated initiative and ability to work well independently
- Demonstrated knowledge of TLC protocols (transfer/lifts for caregivers)
- Demonstrated commitment to professional development
- Physically able to meet the demands as outlined in the Physical Demands Analysis for this position
