Program Overview -- Toronto Seniors Helpline, Community Care
Toronto Seniors Helpline provides access and navigation to services for seniors and caregivers in Toronto via telephone and electronic systems. It facilitates coordination among various sectors including community services, health care, and mental health crisis support. The helpline screens and assesses crisis calls to provide de-escalation, short-term crisis management, and appropriate referrals. It also offers supportive counseling to address issues like isolation and mental health challenges, working in conjunction with other specialized services.
What You Will Do
- Respond to referrals and requests for service via telephone and webchat.
- Conduct initial screening using standardized tools and protocols to determine appropriate responses.
- Assess risk and safety to evaluate callers' risk levels.
- Provide supportive counseling, crisis de-escalation, and short-term case management as needed.
- Use electronic systems for referrals, documentation, and maintaining client files.
- Collaborate with hospitals, primary care, and community service providers to ensure a client-centered approach.
- Maintain accurate statistical data for analysis and reporting.
- Stay updated on community resources and search tools to assist callers.
What You Bring to the Team
- Bachelor Degree in Social Work or related field.
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers or equivalent.
- At least 2 years' experience in telephone counseling/support and crisis intervention, with a focus on older adults and caregivers.

- Cultural competency and ability to communicate with diverse communities.
- Knowledge of community resources and excellent organizational skills.
- Proficiency in MS Word, Excel, databases, and Internet.
- Experience in outreach and public presentations.
- Good judgment, flexibility, and team contribution skills.
- Problem-solving and conflict management skills.
- Ability to follow policies, procedures, and legislation.
