Specialized Outreach Worker, Part-Time (24hrs/wk)

Responsibilities

Initiate contact with referred individuals, assist managers with intake interviews, and assess their suitability for services.
Make referrals to other programs, agencies, and community resources as appropriate.
Assist individuals in planning goals, accessing community resources, skill development, employment, and emotional support. Monitor their well-being and provide feedback to families and stakeholders.
Recognize and analyze potential emergency situations, develop strategies for crisis management, and provide frontline instruction around crisis and community-based interventions, especially for persons with developmental disabilities, FAS/FAE, or Autism Spectrum Disorder.
Help individuals access community resources, coordinate with agencies for housing options, and provide information for informed decision-making.
Advocate for individuals and support their self-advocacy efforts.
Review individuals' progress, document activities, and prepare reports.
Collaborate with other agencies, professionals, ministry staff, and the community.
Develop person-centered, comprehensive Personalized Service Plans aligned with Dr. Shalock's Quality of Life Indicators.
Perform other related duties as assigned.
