What’s the opportunity?
NorWest Community Health Centres is launching an innovative, interdisciplinary initiative: the Homelessness and Addictions Recovery & Treatment (HART) Hub in Thunder Bay. This is a brand-new site purpose-built to support individuals living with complex health challenges tied to homelessness, trauma, and addiction.
We’re hiring a Family Physician to provide primary care in a team-based model. You’ll work alongside nurses, addiction specialists, outreach workers, and mental health clinicians—all aligned to support whole-person care. This role supports either full-time or part-time flexibility depending on your interests.

What you’ll do:
Providing evidence based, client centered substance use care with a strong emphasis on minimizing health risks and improving overall wellbeing for individuals at all stages of substance use.
Deliver trauma-informed primary care to individuals accessing the HART Hub
Contribute to integrated care planning in partnership with community agencies.
Work with clients navigating housing insecurity, systemic barriers and addictions recovery.
What you’ll need:
Current CPSO registration.
Experience or interest in addictions medicine and complex care.
Confidence working within a culturally diverse, equity-focused framework.
Flexibility to adapt care delivery in non-traditional environments.
