Job Description CERTIFIED REHAB AIDE EXTENDICARE -RIVER EAST PERSONAL CARE HOME Applicants are invited to apply for the following position available at our Personal Care Home serving 120 elders from the city and community of Winnipeg, Manitoba. This is a part time permanent position. · DAY PART TIME CERTIFIED REHAB HEALTH CARE AIDE POSITION-PERMANENT - effective immediately Under the direction of the Director of Care, the Rehab aide assesses and supports our Elder’s mobility needs through exercise, ensuring they have appropriate mobility aides and mentors our Health Care Aides in providing daily support for prescribed exercise routine. The Rehab aide works along side the Occupational Therapist/Physio Therapist in assessing our Elder’s rehabilitation needs. RESPONSIBILITIES Assist Elder participation in planned programs which include restorative care, positioning, ambulation, physical functioning and activities of daily living. Facilitates procurement of mobility aides/slings/commodes for Elder’s and families with the support of OT/PT and our vendors. Supports/provides education on lifts and transfers within the Home. Conducts audits on lifts and transfers and provides teachable moments to those as needed. Maintains department records, equipment records, clerical information, inventory control and monthly statistics as required. Monitors and stocks all Rehab equipment in the Home. Participates in the evaluation of care and service provided by the Nursing department to Elders in the Home. Performs all duties in a safe and efficient manner and maintains work area in a clean, orderly and safe manner, adheres to infection control procedures and follows provincial WS&H guidelines/regulations. Participates in team huddles, interdisciplinary care reviews, PATH meetings, committees and meetings as required. Adheres to “Standards of Employee Conduct”, Elder Bill of Rights and Elder Centered Care in the Home. Responsible for the development and upgrading of his/her knowledge and skills. QUALIFICATIONS Preference will be given to Rehab Assistant Certificate and Health Care Aide Certificate and appropriate medical/rehabilitation professional experience. Other appropriate certificates or degrees may be considered. Demonstrates concern and interest in providing quality care to Elders, previous Long Term Care experience an asset. Mentally and physically able to perform responsibilities as outlined. Ability to work independently with minimal supervision and complete tasks in a timely manner. Ability to communicate effectively in English with other staff, Elders, families, and the public in both written and oral form.
