Job Description: Reporting to the Manager of Rehabilitation and Transitional care, the Assessor Coordinator is responsible for case management services for clients in Transitional or Convalescent Care. Core functions include referral/intake, engagement, assessment, care planning, monitoring, evaluation, and termination/discharge through collaboration with family, caregivers, and multi-disciplinary team members. This position liaises with area and provincial services. This position reports to the Manager for daily clinical supervision. This position collaborates with acute care, rehabilitation, home care, long term care, mental health, government and non-government agencies to assess clients' needs and coordinate services. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement."

Valid Class 5 driver's license
Memberships
Saskatchewan Association of Social Workers (SASW)
