Job Description: Position Purpose summary: The Placement Coordinator acts as a liaison to current, potential clients, and their families. This role develops and coordinates the admission process, transfers and discharge of residents; providing support to residents/families throughout their stay at Grace Manor. Accountabilities: Collaborate with Champlain Ontario Health at Home (OHH) to maximize full occupancy and attend meetings, as needed Review all resident applications for admission to determine appropriate level of care; assist with processing of admission and discharge papers Maintain the waitlist Facilitate tours and orientation for general public, potential residents and representatives during the bed offer process Follow-up and facilitate completion of admission checklist Maintain the Internal Transfer List Coordinate internal transfers including the update of documentation Coordinate the proper storage of resident and nursing files Process paperwork for deceased residents Provide Resident Admission Packages for Families and Nursing Complete resident entries and documentation into PCC, RAI Coordinate and ensure the review and revision of the Admission Agreement Coordinate and ensure the review and revision of the Resident Handbook and any other similar information that is provided from time to time to residents/representatives following admission Coordinate resident care conferences involving department managers as required Participate in the following: Daily Team Meetings, OHH/Acute Care/Long Term Care/Ministry of Health Meetings, Memorial Services and special events as required Update and develop new policies for Resident Placement on Policy Manager Family and Resident Relations: Liaison for the Family Advisory Council and attend meetings as required Ensure all Resident Health Cards are updated: renewed, cancelled or address changed Forward new contacts to Family Council Secretary Meet with families and help coordinate smooth transition for admissions Point person for the facilitation of the written complaint process and filing Provide analysis to Executive Director as required Administration: Provide administrative services as required in collaboration with DOC/N, ADOC/N, and Executive Director Support DOC by minute taking for meetings, investigations and reference checks. Ensure the ordering of supplies and medical supplies as required. Maintain knowledge of the purchasing and inventory and ensures deliveries are checked when received. Update and maintains accurate inventory for building Maintain nursing supply room Complete and submit High Intensity and Lab Claims forms in a timely manner and ensures funds received Maintain several mailing lists and coordinates mail-outs Handles and ensures the protection of extremely confidential and sensitive information and material Schedules meeting, prepares agenda and compiles minutes for various meetings involving the Executive Director such as Board of Trustees, Management Meeting and various committees and circulates minutes Coordinates the storage of business, resident, HR and nursing files off-site Prepares catering requests and approves room bookings. Education, Qualifications and Certifications: Completion of relevant post secondary education or equivalent work experience Ability to provide an original copy of a Background Check that is satisfactory to The Salvation Army, in its sole discretion Experience and Knowledge: 1-3 years experience working in a long term care setting 1-3 years experience leading resident/representative-centered meetings Knowledge and familiarity with computer operations and software applications such as Point, Click, Care and RAI-MD Skills and Capabilities: Demonstrated ability to provide support and care for residents and representatives Excellent communication skills, organization and attention to details Demonstrated commitment to resident centered elder care, treating others with respect and dignity Ability to work cooperatively and effectively with staff, residents, visitors, and the public The ability to achieve objectives, ensuring multiple priorities and tasks are appropriately carried out Intermediate computer skills in Microsoft Word, PowerPoint and Excel Excellent written and verbal communication skills in English. Bilingual English/French considered a strong asset Compensation: The target hiring range for this position is $23.28 to $29.10 with a maximum of $34.92. Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
