Summary
Reporting to the Social Work Site Leader, and receiving guidance and support from the Professional Practice Leader and/or Research, Education and Practice Coordinator, the Social Worker:
provides therapeutic clinical counselling services and conducts comprehensive biopsychosocial assessments
maintains accurate and confidential patient/resident records in accordance with professional standards
offers consultative and direct services to patients/residents, families, staff, and community health professionals regarding available resources
leads and coordinates interdisciplinary discharge planning to ensure continuity of care
develops and delivers in-service education and presentations for staff and participates in committees and team meetings
maintains current knowledge of social work practices, research, and professional developments
What You Bring
Education:
Master’s Degree in Social Work from an accredited university, plus one (1) year of recent related experience in a designated program area, or an equivalent combination of education, training, and experience. Current full registration with the British Columbia College of Social Workers (BCCSW) required.
A Bachelor of Social Work degree from an accredited School of Social Work, combined with an equivalent combination of education, training, and experience, may be considered. Current full registration with the BCCSW required.
Skills and Abilities:
Demonstrated ability to communicate effectively with patients/residents, families, staff, physicians, and other healthcare providers.
Knowledge of legislation, government policies, and procedures related to healthcare and social services.
Demonstrated ability to gather and analyze relevant data and perform comprehensive biopsychosocial assessments.
Demonstrated skill in conducting patient/resident assessments and developing care plans.
Knowledge of other healthcare disciplines and their roles in patient/resident care.
Ability to elicit and interpret information that contributes to a full understanding of the patient/resident’s situation and underlying issues.
Proven ability to work collaboratively as part of an interdisciplinary team.
Demonstrated ability to address and resolve conflict situations effectively.
Physical ability to perform the duties and responsibilities of the position.
Strong organizational skills with the ability to work independently, manage priorities, and meet deadlines.
Proficiency in operating related computer and clinical equipment.
Ability to participate in and contribute to research and quality improvement initiatives.
Demonstrated ability to provide supervision, mentorship, and teaching to students and peers.
Ability to facilitate and influence group discussions, meetings, and shared decision-making processes.

Your Day to Day:
Provide therapeutic clinical counselling to patients, residents, families, and groups to identify issues, set treatment goals, and facilitate positive behavioral and emotional change.
Conduct comprehensive social work and psycho-social assessments, including interviews and data gathering on family, cultural, environmental, and financial factors.
Maintain accurate and detailed patient records, including assessments, progress notes, and reports, in accordance with established standards and regulatory requirements.
Participate in interdisciplinary care teams to contribute to treatment and discharge planning, liaising with community partners to coordinate services and ensure continuity of care.
Offer consultation and direct support to patients, families, staff, and community professionals regarding available community resources and access to financial or social assistance.
Support patients, families, and teams in understanding relevant legislation and processes, including substitute decision-making, medical assistance in dying, and adult guardianship.
Engage in program evaluation and quality improvement by contributing a social work perspective on patient care, service effectiveness, and system efficiency.
Develop and deliver staff education, presentations, and training sessions related to social work practice and patient care initiatives.
Participate in committees, research, and special projects to enhance service delivery, identify gaps in care, and implement evidence-based practices.
Provide supervision and teaching to social work students, fostering learning and skill development in clinical settings.
