The Substance Use Service (SUS) (Addiction Medicine program) offers a holistic, trauma-informed approach to the assessment and support of people who use alcohol, opioids, benzodiazepines, cocaine, methamphetamines, cannabis or other substances. Patients range in age, social and income backgrounds and often present with concurrent mental health challenges. The team, comprised of physicians, a nurse practitioner, addiction service workers, peer support a worker, psychiatrist and part-time social worker, supports a spectrum of person-centered goals including harm reduction and abstinence.
An exciting Temporary Part-time opportunity as a Registered Nurse exists in Substance Use Service (SUS) reporting to the Clinical Manager.
Summary of Duties, but not limited to:
Complete the initial assessment and develop a care plan for patients seen in the RAAM clinic and Substance Use Service, working closely with the interdisciplinary team. This includes gathering substance use, medical, point of care urine drug testing, mental health, and psychosocial history, along with a review of current medications
Perform vital signs monitoring and withdrawal assessments including tools such as the CIWA-Ar and COWS, documenting and communicating findings to the interdisciplinary team
Coordinates patient flow and care with the rest of the RAAM/Substance Use team
Provides supportive counselling to RAAM patients
Provides individualized education on substance use, harm reduction strategies, safe medication use, infection prevention (e.g., HIV/HCV testing, safe injection practices, vaccinations), and self-care during recovery
Provides patients with harm reduction education and supplies
Assists with EMS calls/ transfer of patients to nearby emergency departments if medically necessary
Co-ordinates patient care in collaboration with the patient/family and members of the interdisciplinary team, including referral to WCH or external services/community agencies
Co-ordinates discharge planning in collaboration with the patient/family and members of the interdisciplinary team
Advocates for patients by mobilizing resources and identifying and addressing gaps in service to meet supportive care needs of patients, such as primary care providers
Support patients referred to WCH’s Acute Ambulatory Care Unit for withdrawal management, collaborating closely with the interdisciplinary teams in both SUS and AACU
Assist the SUS Medical Secretary with questions related to prescriptions and/or patient bookings as required
Assists with triage of new referrals
Documents according to the College of Nurses, Documentation Standards
Participates in program development and quality improvement initiatives related to addiction care, workflows, and nursing practice
The Registered Nurse (RN) is responsible and accountable for nursing care and will practice to their level of competence as outlined in the Standards of Practice by the College of Nurses of Ontario
Works collaboratively with nursing colleagues, physicians, allied health team, patient/family to provide integrative patient- focused care for patients in the Substance Use Service Program

Qualifications
Bachelor of Science in Nursing (BScN) preferred
Current registration with the College of Nurses required
BCLS certification required
Minimum of two (2) years’ experience in a comparable nursing role, preferably in an ambulatory care setting
Experience with addiction medicine care in an ambulatory clinic, withdrawal management setting or within a hospital setting an asset
Must be able to work independently and collaboratively in a team environment
Ability to work well under pressure and use good judgment in assessing difficult situations
Demonstrated excellent verbal and written communications skills
Demonstrated knowledge and skill in critical thinking and decision-making
Strong MS Office application skills including Word, Excel and Outlook
Ability to produce high quality work in accordance with Hospital standards
Excellent organizational and time management skills that compliment well developed interpersonal skills, including the ability to handle multiple duties
Proven attendance and punctuality.
Professional behaviour and communication that meets the standards of the professional regulatory college or association, as applicable, and the standards of Women’s College Hospital
