The Alzheimer Society of York Region is seeking an enthusiastic and resourceful Social Worker to join our Programs and Client Services, Support and Education Team. The successful candidate will report to the Social Work Manager.
SOCIAL WORKER -- FULL TIME ONE YEAR CONTRACT
The Alzheimer Society of York Region (AS York) helps improve the quality of life for people diagnosed with Alzheimer's disease and related diseases. We provide supportive counselling and education opportunities for individuals and families living with dementia. AS York also provides respite to caregivers and social activities for persons diagnosed with cognitive impairment through our D.A.Y. Centres. A non-profit, community-based agency, AS York offers programs and works in partnership with several community organizations across the Region.
Key Responsibilities:
- You will provide direct client services to individuals and families living with Alzheimer's disease and related diseases (individual/family/group) virtually and face-to-face.
- Clinical services may include psycho-educational interventions for caregivers, disease education, supportive counselling, stress and behaviour management education, bereavement support, and advocacy.
- Assessment of the needs of the client, including assessment for respite day program services.
- Knowledge of community services and appropriate referrals.
- Documentation, keeping current and accurate records.
- Collect statistics and qualitative data on the program and provide written reports, as required.
Qualifications:
- BSW or MSW, eligible for certification with the OCSWSSW.
- Experience working with diverse cultures, older adults, and persons with cognitive impairment.
- Fluency in English both oral and written.
- Ability to work independently and with members of an interdisciplinary team.
- Computer competency required.
- Ability to work flexible hours, some evenings and Saturdays as required to a maximum of 35 hours per week.
- Valid Ontario Driver's License and access to a vehicle in good running order.
- U-First \& GPA Certifications, CPR/First Aid Certification would be an asset.
Start Date: Immediately
Salary Range:$50K-$55K/Annually
