Position Summary
The Toronto Rehabilitation Institute, Canada's largest adult rehabilitation hospital, integrates innovative patient care, research, and education to help individuals recover from injuries, illnesses, or age-related conditions. It operates across four sites, including University Centre, Lyndhurst Hospital, Toronto Rehab's Cardiac and Stroke Centre, and the Bickle Centre. The LTLD/Geri unit is a 48-bed inpatient unit providing rehabilitation for patients with multiple co-morbidities. Our goal is to facilitate safe and effective transitions to the community through collaborative discharge planning.
Duties
- Accept patient assignments as needed
- Plan, manage, evaluate, and coordinate comprehensive nursing care within an interdisciplinary team
- Utilize theoretical and empirical knowledge to plan and implement patient care
- Educate patients, families, and community providers about rehab nursing
- Participate in team care planning, rounds, and family conferences
- Provide leadership, supervision, and mentorship to students and new staff
- Engage in professional development and hospital initiatives
- Ensure compliance with patient safety policies and procedures
Qualifications
- Member in good standing with the College of Nurses of Ontario
- Experience in Low Tolerance Long Duration rehab preferred
- Certification in gerontology or rehab nursing preferred
- Specialty certification (Canadian Nurses Association) preferred
- Basic Cardiac Life Support (C level) certification required
- Membership in RNAO or OARN (SIG) preferred
- Strong understanding of nursing concepts and rehab care models
- Excellent clinical decision-making, communication, and leadership skills
- Ability to work independently and in teams
- Knowledge of rehab nursing standards and healthcare legislation
- Commitment to ongoing professional development
- Ability to provide clinical rationale for patient care
- Willingness to work rotational shifts
- Proficiency in computer skills or willingness to learn
- Strong health assessment skills
